Job Description Summary

As part of the Hospital and Home Admissions Teams, Pre-Admission Support Specialist (PASS) supports the admission process and facilitates speed to care by providing a range of pre-admission and admission services on behalf of patients, families, and caregivers. The Pre-Admissions Support Specialist also collaborates with community partners to resolve barriers to admission and placement. The position is also responsible for providing information and education regarding Community Hospice & Palliative Care services and philosophy. Consults with admission nurses and assists in resolving Advance Directives questions, goals of care discussion, discharge planning and contributes to a seamless transition of care.

Primary Responsibilities:

  • Provides patients, families and caregivers information about Community Hospice services and philosophy and other community related resources as appropriate.
  • Assists with obtaining Community Hospice & Palliative Care consents.
  • Provides information regarding Health Care Surrogate, Health Care Proxy and other Advance Directives.
  • Assists as directed in arranging/coordinating and supporting patient care at the time of admission and/or when discharging from one care location to another.
  • Documents patient/family contacts
  • Establishes, develops, and maintains relationships with area social workers, case managers and community agency staff in order to network and facilitate admission and placement of patients.
  • Provides emotional support to family for patients in active dying process. May make bereavement calls for patients who die while hospitalized.
  • Attends in-service trainings, workshops, conferences, agency and departmental meetings.
  • Complies with appropriate professional code of ethics in delivering services to patients and families.
  • Other duties as assigned.

Required Licenses/Certifications:

Current State of Florida driver’s license with reliable transportation and driving record acceptable to company insurance carrier.

Qualifications:

  • Field of case management, social work or related field
  • Three (3) years of related healthcare experience may substitute for above minimum requirements.
  • Basic proficiency utilizing Microsoft Office software products

Knowledge of:

  • Hospice services and philosophy.
  • Admission procedures.
  • Medicaid Application Process.
  • Community resources.
  • Referral and placement protocols for Independent Living, Assisted Living and Long Term Care facilities in the service area.
  • Must be willing and able to flex hours and access all locations of care facilities including hospitals and other health care facilities in order to meet admission and referral requests.
  • Must possess excellent verbal and written communication skills; strong critical thinking ability.
  • Ability to work within a framework of multidisciplinary teams
  • Ability to work in stressful and sensitive environments while maintaining professional demeanor with patients, families and referral sources
  • Ability to research, analyze and interpret information
  • Ability to organize and prioritize work schedule
  • AHCA Level II Background Screening require

Job Type: Full-time

Pay: $18.00 - $22.00 per hour
