We taking an old-fashioned, family-centered and personalized approach to care while demonstrating the best of today’s treatment through evidence-based clinical guidelines. We have great relationships with insurers and are high performing on clinical measures and outcomes.

We are currently seeking a FULL-TIME Family Nurse Practitioner or Physician Assistant to join our team at our new Anthem location!

Our Primary Care Providers will enjoy practicing population health based medicine across the lifespan in a non-rushed and provider friendly environment. A typical day consists of 16 patients, focusing on delivering comprehensive care in a family centered environment. Your primary responsibilities will include conducting the following:

• Well visits to include Well Woman Exams, Complete Annual Physicals, AWV
• Same day sick visits including fever, UTI, headache and allergies
• Chronic disease management including diabetes, hypertension, COPD
• Population Health Management, preventive care, healthy living, disease prevention

Though NextCare Primary Care is a part of NextCare Urgent Care, it will operate as a primary care office and see patients by appointment only. You will not be providing urgent care appointments in this position or working at a facility that provides urgent care.
