Please note that individual total compensation for this position will be determined at the Company's sole discretion and the wage range for this role considers a wide range of factors including but not limited to skill sets; experience and training; licensure and certifications; and other business and organizational needs. At SNAHC, it is not typical for an individual to be hired at or near the top of the range for their role and compensation decisions are dependent on the facts and circumstances of each case. A reasonable estimate of the current range is $46,259.28-$62,586.08


Position Summary:

The Community Health (CH) Program Coordinator reports to the CH Program Manager. This is the integral position to the operations of grant programs focused on planning, developing, improving, and delivering mental health/substance use prevention use activities to Native American youth and their families. Working collaboratively with the CH Program Manager, the Program Coordinator will be responsible for leading, coordinating, and ensuring execution of required activities for community health programming. This position will receive training and support from the CH Department and contracted evaluation team to fulfill these responsibilities.

Essential Functions:

  • Support the establishment and implementations of age-appropriate, culturally responsive programs, groups, activities, and interventions related to mental health, trauma, grief and loss, and substance use and alcohol prevention
  • With guidance from the Program Manager, coordinates and implements grant funded prevention services and activities.
  • Assist with grant applications, reporting, and evaluation activities
  • Assist in partnerships that engage key stakeholders and community members in addressing community health priorities and meeting grant deliverables
  • Supports planning for and onsite staffing of SNAHC Gathering of Native Americans (GONA) and other community health events.
  • Ensures SNAHC services and programs are delivered in a culturally appropriate manner
  • Desire to work closely with youth and families

  • Must have excellent organizational and computer skills including experience with word processing and spreadsheet skills.
  • Complies with all state and federal laws and regulations, as they pertain to the position including; HIPAA, sexual harassment, Scope of Practice, OSHA, SCAR reporting, etc.
  • Other duties as assigned.

Minimum Qualifications:

  • Experience coordinating, implementing, or monitoring federally funded grant programs.
  • Must be able to lift 40 lbs and be responsible for the set up and breakdown of outreach booth and equipment
  • Must have a valid California Driver’s License with current auto insurance and reliable transportation.
  • Must be able to work nights and weekends as needed
  • Experience in an FQHC setting as a member of a multi-disciplinary team preferred.
  • Knowledge of traditional, cultural and spiritual practices of the diverse AI/AN community, as well as ability to work with other racially, culturally, ethnically, and financially diverse populations

Preferred Qualifications:

  • 2 years of experience coordinating, implementing and/or monitoring community and/or grant funded programs
  • 2 years of experience in direct service in youth prevention.
