JOB SUMMARY
The Program Coordinator is responsible for managing the Health Navigators and Peer Support Specialists
ensuring quality outcomes are achieved, developing new collaborations with the community, monitoring
program outcomes and ensuring program services are being delivered according to the contract. This
position will also be responsible for marketing of the program and community awareness.
ESSENTIAL DUTIES AND RESPONSIBILITIES
1. Supervise daily activities of Health Navigators and Peer Support Specialists to include the implemented
activities at targeted intervals of time and assist with the collection of program data for evaluation.
This position will be supervising staff in the Northshore and New Orleans areas. Travel to both
locations several times per week will be required.
2. Monitor services to ensure they are being delivered according to contract.
3. Assist with providing education core skill recovery classes.
4. Perform quality assurance checks on consumer files and Care Logic EHR.
5. Assist with locating resources for linkage to care for program participants.
6. Assist with acquiring additional trainings and certifications for program staff members for additional
enrichment of the program.
7. Promote the program through marketing and community awareness to increase referrals of eligible
individuals into the program.
8. Performs related duties as assigned.
KNOWLEDGE, SKILLS AND ABILITIES
This position requires the incumbent to have completed education at a minimum of a Bachelor's degree in a
human service related field, Master s preferred. This position should also have at least one year of care
coordination and substance use experience. Great organizational skills are a must. Knowledge of HIV and
testing is a plus. Experience working in a community setting is preferred; Have a demonstrated willingness
to be responsive to the needs of the target population, excellent verbal and written communication skills and
the ability to function as part of a team; can handle a flexible work environment; have strong decision making
skills.
PHYSICAL REQUIREMENTS
