POSITION SUMMARY

The CHW (Community Health Worker) program coordinator will serve as a bridge between TCA Health, community-based organizations, and patients, by coordinating health services and programming within the TCA Health service area. The CHW program will work across targeted neighborhoods in Chicago’s South Side and Southern Suburbs [SO1]to address social determinants of health and access to healthcare.

ESSENTIAL DUTIES AND RESPONSIBILITIES (include but not limited to the following);

§ Supervise CHWs day to day activities by providing intensive and ongoing supervision to the CHW team as they engage a targeted patient/community member population around health issue areas.

§ Provide ongoing evaluation of CHWs skills; developing of tools to document CHW’s progress; providing regular feedback communications; ensuring CHW’s are timely in attendance and job functions; monitoring and assessing progress on program.

§ Support CHWs assigned to the team on a regular basis with 1:1 meetings, coaching sessions, directing CHWs to support materials, developing support materials, identifying necessary CHW training needs, and developing program materials/tools to support CHWs in their job.

§ Conduct periodic performance evaluations of direct reports, maintain timecards and schedules of the CHW team.

§ Develop linkage agreements, and or geographically specific resource lists outlining community-based resources that reduce barriers to care, and or address social determinants of health.

§ Build team and organizational capacity by hosting in-service and all staff trainings on health issues, and promoting health education in the community and community based organizations.

§ Ensure data and client tracking and reporting, including conducting regular monitoring assessments on data entry status, developing quality improvement protocols, and conducting data quality data checks. This also includes monitoring and summarizing CHW tracking of their day-to-day activities and reporting them to the Director of Population & Community Health.

§ Assist in developing program models and curriculum around new CHW programs and administer assessments and assist patients/families to identify needs, provide basic health information, and make appropriate referrals to community services.

§ Assists with Community Outreach Program

a. Assists in assessing community health related needs.

b. Provides technical assistance and serves as resource on health issues.

c. Advocates for underserved population in service area.

d. Networks with community agencies and organizations to promote services offered by TCA Health, Inc.

§ Follows role assignments aligned with the Patient Centered Medical Home Standards, and participates in all Patient Centered Medical Home efforts as assigned.

EDUCATION, TRAINING AND EXPERIENCE

§ Associates Degree in Nursing, Health, or Human Services, BS in Community Health, or combined equivalent of work experience, education and certifications.

§ 1+ years of experience in Health Education, Counseling, or Human Services.

§ Certification in a disease discipline (preferred)

Job Type: Full-time

Pay: $50,000.00 - $55,000.00 per year
