Responsibilities

POSITION SUMMARY:

This position is 24 hours per week.

The SoulMates Program Coordinator II (PC) is a staff member of the Division of Social Work within the Department of Psychosocial Oncology and Palliative Care. The SoulMates program supports women undergoing breast cancer treatment through peer mentorship. The mission of the SoulMates program is to enhance the comprehensive care of the Susan F. Smith Center for Women’s Cancer for individuals diagnosed with breast cancer by matching them with volunteer peer mentors for support, education, and practical tips. Reporting to the Director of Clinical Operations in Psychosocial Oncology and Palliative Care, the PC II will carry out the primary PC duties and responsibilities as outlined in this job description. Additionally, the PC II will assist with the ongoing implementation, follow-through, and evaluation of the SoulMates Program.

PRIMARY DUTIES AND RESPONSIBILITIES:

  • The PC will interact with patients and mentors by phone, telehealth, or in person as needed/ directed.
  • The PC will coordinate peer matching per protocol established within the Division of Social Work.
  • Per protocols established through the Division of Social Work, the PC will assess the services individual patients need and then connect and follow-up on referrals as needed/directed.
  • Must maintain patient confidentiality and compliance with HIPAA, while reporting patient concerns per protocol as established within the Division of Social Work.
  • The PC will be responsible for keeping the SoulMates databases up-to-date.
  • The PC will liaison with Volunteer Services, to onboard peer mentors and to ensure compliance with established Volunteer Services protocol.
  • The PC is responsible for reviewing the weekly peer mentor reports from the REDCap database and to report any patient related concerns per protocol established within the Division.
  • The PC is responsible for organizing community meetings, as appropriate, to support the SoulMates program, including assessing interest and engagement in priority topics, and scheduling guest speakers to address such topics. The PC attends all community meetings.
  • Working in close collaboration with a clinical social work liaison, the PC is responsible for coordinating monthly mentor meetings to provide support and ongoing training.
  • The PC is responsible for engaging and communicating with internal and external stakeholders/collaborators of the SoulMates Program.
  • The PC is responsible for additional administrative responsibilities as needed.

  • BA/BS required.
  • Must have excellent written and verbal communication and interpersonal skills.
  • Must be able to communicate with patients, mentors, and staff effectively, with sensitivity.
  • Strong organizational skills and ability to set priorities and work independently required.
  • 1-2 years of administrative experience preferred.
  • Analytical skills; computer proficiency.
