The HIV/Sexual Health Program Coordinator will oversee and enhance the provision of HIV/Sexual Health prevention services for people living with HIV, Hepatitis C, and Prevention Services (PrEP and PEP). They will be responsible for coordinating outreach to persons in the community who may benefit from HIV/HCV/STI prevention services and supporting their retention in care and services. The position reports to the Sr. Practice Administrator – David Powell and work alongside the Sexual Health leadership team to establish care guidelines, workflows, policies and procedures and relationships with relevant internal and external stakeholders, to track and improve HIV testing rates as well as the care outcomes for people at risk of HIV. The Program Coordinator – Sexual Health performs community outreach activities to promote understanding and support HIV/HCV/STI testing. This position will act as community liaison and referral services regarding HIV/STI/HCV testing, education, and outreach to internal and external partners.
Responsibilities:

- Work with vulnerable populations including LGBTQ+, people with mental health and/or substance use disorders and/or underprivileged/underinsured.
- Work in clinics and communities, as needed, providing education to clients regarding sexual health.
- Educates the community regarding sexual health and public health services including testing, education, outreach for HIV, STIs, PrEP and PEP.
- Strategic development of community partnerships to enhance HIV/HCV/STI prevention capacity in the region
- Coordinate and oversee the aggregation, verification, and reporting of all community program daily, weekly, monthly, and quarterly data into databases by community level

- Identify gaps and challenges impacting Rapid ART services and conduct community level QI solutions and/or
- Submit timely, accurate and quality reports on community HIV/HCV program activities performed.
- Assists with developing clinical and administrative procedures and policies regarding sexual health services.
- Monitor, track, and deliver HIV-Opt Out material and test kits to clinical sites within organization.
- Champion facility – community integration in the implementation of testing, education, and outreach.
- Maintains privacy and confidentiality of all patient information.
- Assist HIV Grant Manager with monthly grant time and attendance.
- Assist HIV Grant Manger with monthly data for HIV – Opt Out Grant and HCV Routine Screening.
Knowledge/Skills/Abilities:
- Deep understanding of the intersections of race, trans/non-binary identity, and class as they pertain to HIV prevention, advocacy, and education.
- Knowledge of and sensitivity to the HIV/AIDS patient population, and/or an eagerness to learn about this community.
- Commitment to advocacy on behalf of persons living with HIV/AIDS (PLWH/A).
- Ability to establish and maintain effective public and working relationships with culturally diverse populations from a wide range of life circumstances and backgrounds.
- Excellent judgment, including ability to handle sensitive/confidential information.
- Effective interpersonal/communication skills.
- Self-Starter with initiative; Highly organized with attention to detail
- Robust and dynamic problem-solving and critical thinking skills.
- Travel 30% of the time between CUC clinical sites and community events.
- Ability to work in a team setting including strong communication and follow-up skills.
- Self-motivated and able to work independently
- Demonstrated ability to accurately and quickly gather data, compile information, and keep accurate records
- Ability to coordinate and collaborate successfully with other groups and organizations.
- Ability to manage relationships and effectively communicate with team members and agency officials, grantors, and funders.
- Experience working in a fast-paced environment; Ability to multitask.
- Comfort with public speaking; providing training, presentations at community meetings, and conferences.
- Experience in implementing public health programs and capacity building
- Proficiency in Microsoft Word, Excel, Access, and PowerPoint.
- Excellent written, oral, interpersonal skills, organizational skills, and project management skills.
MINIMUM EDUCATION: Baccalaureate degree in social work, health care, or a human service discipline from an accredited college or university.
