Position Overview:
The primary purpose of your job position is to oversee the daily participation, documentation and running of STP groups for your assigned clients.

Key Responsibilities:
- Lead groups in the following areas: behavior interventions, self-help skills, interpersonal relationships, prevocational preparation and prerelease planning.
- Provide group, individual and family counseling.
- Maintain on-going progress notes on clients.
- To assess and monitor the mental health and social needs of clients.
- Report change in conditions or a decline in behavior to IDT.
- Attend IDT meeting to assimilate and disseminate pertinent information to management and other department heads.
- Oversee client attendance and review programming calendar to ensure clients are attending at least 27 hours of scheduled programs a week.
- Assist in maintaining individual client health record of each client with assessment and input as appropriate from the health professionals involved including but not limited to: meeting behavioral objectives.
- Assist with development and implementation of an individual, written care plan based on identified clients’ needs.
- A review and updating of the care plan as necessary not to exceed 3 months.
- Assist with plan of care which shall be reviewed approved, signed and dated by the attending Physician.
- Assist with monthly written progress notes in the record written by members of the staff providing services, the notes shall be specific to the needs of each client and the program objectives and plans.
- Assist with reassessment and summary of the progress of the clients the appropriateness of program objectives and the success of the plan.
- Assist with initial evaluation and assessment by facility staff of their medical, nursing, dietetic, social and physical needs within 15 days of admission unless evaluation has been completed by referring agency within 30 days prior to admission.
- Ensure individual clients admitted will had a psychological evaluation and assessment by appropriate discipline within 45 days of admission.
- Ensure patients/clients rights, program requirements, treatment services and staff are maintained as required.
Requirements:
- Must have a Bachelor’s degree in Psychology, Social Work or related field.
- Must be able to read, write, speak, and understand the English language.
- Must possess the ability to make independent decisions when circumstances warrant such action.
- Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel, and the general public.
- Must possess leadership and supervisory ability and the willingness to work harmoniously with and supervise other personnel.
- Possess knowledge of direct resident care services which ensures the safety, comfort and protection of residents.
- Must have patience, tact, a cheerful disposition and enthusiasm, as well as the willingness to handle difficult residents.
- Must be willing to seek out new methods and principles and be willing to incorporate them into existing practices.
- Must be able to relate information concerning a resident's condition.
- Must not pose a direct threat to the health and/or safety of other individuals in the workplace.
