JOB DESCRIPTION

Provides effective administrative oversight of all processes and services related to Williams Colbert Expansion to promote access to services and sustained community placement for individuals transitioning to the community from NF/SMHRF/IMD care. AHC will have oversight over subcontractors for performance and data reporting, and financial reporting; QA will provide leadership as an agency representative. QA oversees all aspects of implementation of services to Class Members and conducts wellness visits with Class Members in their homes. Monitors documentation related to the transition process and participant care. Monitors delivery of behavioral health services and ensures continuous quality improvement in relation to contract deliverables and standards. Assists in development of risk mitigation strategies and participates in crisis management services. Provides clinical and administrative oversight. Represents agency and programs effectively in the community, and assists in building collaborative relationships with other providers, facilities, and entities.

Requirements

  • Master’s degree in counseling, social work, psychology or related social services area from an accredited university or college.
  • LCSW/LCPC preferred.
  • Two years of experience in community behavioral health setting (Required).
  • Experience working in diverse populations (Required).
  • Flexibility to work on weekends, evenings, holidays during emergencies.
  • Basic computer skills. Valid IL driving license.
  • Licensed use of a car, current car insurance and driving record acceptable to Agency insurance carrier.
