Summary of Major Job Functions, but not limited to:

• Reviews documentation including; Oasis SOC/Transfer/ROC/Recertification/Discharge, discipline discharge summary, nursing revisit notes, Initial Therapy and Social Work progress notes submitted by all home health providers to evaluate regulatory compliance and quality assurance.

• Assists Clinical Supervisor in identifying clinicians' with documentation deficiencies.

• Evaluates clinician's documentation. Assists Clinical Supervisor with the implementation of staff development processes.

• Serves as resource to clinical staff on documentation errors or questions.

• Audit charts to ensure completeness of chart and in compliance with regulatory entities.

• Participates in QAPI activities as per direction of Clinical Supervisor.

• Attends educational seminar for CMS updates as directed by Clinical Supervisor.

• Coordinates and communicates with various team members of QA team including remote coders.

• Monitor clinical staff with timely submission of documents.

• Coordinates and communicates with clinical staff for any missing or incorrect information on documents and follow up any corrections made by clinicians.

• Escalate to Clinical Supervisor any barriers related to submission of visit documents, correction of errors, missing required documents, or completion of physicians’ orders.

• Attends departmental staff meetings, case conferences and in-services as scheduled.
• Accurately assigns ICD-10-CM codes from the care plans, and other applicable documentation in accordance with official coding guidelines.

• Performs related work in this classification routinely and clerical work unrelated to QA as directed by Office Manager or Clinical supervisor.
