Job Functions

  • Conduct and coordinate the development and completion of the resident assessment (MDS) in accordance with current regulations and guidelines that govern the resident assessment.
  • Develop preliminary and comprehensive assessments of the nursing needs of each resident, utilizing the forms required by current rules and regulations and facility policies.
  • Certify completion of OBRA (Admission, Quarterly, Annual and Significant change) and PPS assessments by signature and date.
  • Ensure that all members of the assessment team are aware of the importance of completeness and accuracy in their assessment functions and are aware of the penalties for false certification. Hold them accountable for accuracy and completion.
  • Compile data from nursing staff and all Care Team Members to determine need for resident re-assessment.
  • Coordinate the development of a written plan of care (preliminary and comprehensive) for each resident that identifies the problems/needs of the resident, indicates the care to be given, goals to be accomplished, and which professional service is responsible for each element of care.
  • Ensure that the care plan includes measurable objectives and timetables to meet the resident’s medical, nursing, mental, spiritual, and psychosocial needs as identified in the resident’s assessment: and ensure that the care plans are updated and current.
  • Monitor regulatory compliance of specified completion dates of all assessment and care plans.
  • Plan and develop all resident care plans within 21 days of admission and update as needed and minimally every quarter.
  • Review resident medical records to ensure the plans of care address resident needs or concerns and ensure interventions to address these are implemented.
  • Assist Activity Staff in developing activities required for the resident assessment and plan of care.
  • Ensure that OBRA and PPS (scheduled and unscheduled) resident assessments and care plan reviews are made on a timely basis. Coordinate scheduling of MDS assessments with services provided in order to capture the most appropriate RUG level that reflects the resident’s condition.
  • Ensure that a complete resident assessment is conducted within fourteen days of identification of a significant change in the resident’s condition.
  • Coordinate the review and revision of the resident’s care plan by the interdisciplinary team after each quarterly review or other assessment, assuring that the care plan is evaluated and revised each time an assessment is done or when there is a change in the resident’s status.
  • Ensure that each portion of the assessment is signed and dated by the person completing that portion of the MDS.
  • Provide Direct Care as requested.

Education: Must possess, as a minimum, a Nursing degree or diploma from an accredited college, university or program
Experience: Minimum of 1 year long term care experience as an RN Supervisor

  • MDS 3*.0 and PDPM Preferred

Certificates/Licenses: Must be licensed as a Registered Nurse in the Commonwealth of PA
Computer Skills: Basic - word processing, spreadsheet, Internet usage for research/resources
