Silver Health CARE is looking for an RN or LPN to do a combination of both Administrative and Clinical work. Become a leader in the transitions and incorporations of the aspects of Population health; AWV, quality measures, CCM, etc. Create and implement effective programs to reach goals such as training, education, and quality improvements.

Position Requirements

· Current licensure as a Nurse (LPN or RN)

· Previous experience in caring for chronic disease patients required

· 2 years’ experience in provider practice or clinic health setting preferred

· Possesses strong clinical assessment and critical thinking skills necessary to develop a comprehensive plan of care appropriate to patents with complex medical, emotional and social needs

· Can pay attention to detail and accuracy of documentation in records

· Has the ability to work in a high-volume caseload environment and deal effectively with rapidly changing priorities

· Demonstrates evidence of effective organization, leadership communication, education collaboration, and counseling skills

· Previous Care Coordination, Case Management, Home Health or Behavior Health experience preferred

· Previous experience with mobilizing community resources, navigating patients through the healthcare continuum, and working with disparate populations preferred

· Experience managing, supervising, or leading

· Ability to speak Spanish preferred

· Ability to identify and implement appropriate patient communication strategies

· Previous experience with health IT systems and data reports preferred

· The demonstrated ability to work constructively with all disciplines related to caring for patients within the community

· Ability to teach to a wide variety or clinical staff

· Able to recognize opportunities for improvement, create program proposals and implement changes

· Demonstrates effective delegations skills to streamline operations workflows and optimize inter-office resources

· Ability to speak clearly and concisely on telephone

· Have an understanding of Population Health

Responsibilities

  • Lead Global Risk Support Team to assess/plan/implement care plans
  • Identify and develop/implement plans to address patient challenges in effectively fulfilling clinical care plans
  • Communicate progress to patients’ clinicians and group CMO
  • Lead team in coordinating care between Clinicians, outside entities (local regulatory/social support/clinical partners), patients, and families
  • Optimize care to be time and cost effective
  • Maintaining care management records for review by clinicians and CMO

Manage care team- scheduling, quality control, and performance improvements, competency training

Job Type: Full-time

Pay: From $75,000.00 per year
