Registered Nurse – Clinician owned and managed home health agency

THE POSITION

Advantage is seeking a highly motivated, experienced RN to provide nursing and geriatric care management services for its patients as a RN Case Manager.

A successful candidate will have at least one consecutive year of current experience working as a Registered Nurse. We are seeking individuals that are:

• Team players willing to help, motivate and teach patients as well as fellow co-workers.
• Good communicators with our patients, caregivers, and care continuum team (internal staff, co-treating disciplines, social workers, discharge planners and physicians).
• Organized and able to manage a case load of 20 to 25 patients in an autonomous work environment.
• Registered Nurses (Associates or BSN) licensed with the Ohio Board of Nursing.

THE LOCATION

We are a multi-county home health agency with a strong management team comprised of clinicians. RN Case Managers travel to patient’s place of residence to provide care.
