Tender Loving Family Care (TLFC) is looking for a compassionate Registered Nurse to join our growing team of Home Care Professionals!

The RN's main goal in Home Care is to assess the Clients for the correct services, continue proper care, assist with a Plan of Care, maintain a healthy relationship between the Agency, Client/family, and our Caregivers.
Assist with Employee supervision, or emergency situations, effectively plan In-services for staff members, update client files, maintain current doctors' orders, and much more!

This position will require that you are available nights and weekends as need dictates.
Experience performing UAS assessments is a plus.
Responsibilities :
- Visit client in need of short/long term care
- Perform Assessment for potential client
- Conduct face to face assessments according to state guidelines, policies, procedures, and protocols
- Review consumer medical documentation and or health referral forms, as relevant to the client
- Enter assessment data into electronic assessment form and transmit, as required
- Emphasize continuity of care, thus reducing or eliminating fragmentation, duplication, and gaps in treatment plan
REQUIRED Licensure/Certification: 2+ years of Home Care experience NYS Registered Nurse (RN) license

Pay: $44,170.00 - $79,751.00 per year
