For over 25 years, Pathways Hospice & Palliative Care has been enhancing the lives of patients and their families by combining the most advanced medical care with a focus on compassion and comfort. We are seeking a full time Hospice RN Case Manager for the West County area. As an RN Case Manager you will:

• Serve as the team leader of an interdisciplinary group of professionals who, together with the patient, develop aplan of care that best meets the needs of the patient and his/her loved one
• Work with patients in the home or nursing facility setting
• Be provided a laptop, software and individualized training for optimal documentation
• Attend a team meeting weekly where you work with the support of other professionals to problem solve and update the patient's plan of care
• Visit your patients 2-3 times a week on average to assess and coordinate care of the patient
• Receive ongoing training and educational opportunities

The ideal candidate for this position is a dedicated professional with excellent assessment and organizational skills who prefers to work in the home/nursing home setting where care is provided in the patient's own environment. Hospice, oncology, med surgery or long-term experience is preferred, as is computer proficiency. Minimum of 1 year nursing experience required. Current RN license and care insurance are required.
