We are currently looking for Resident Care Provider – Med Tech that will serve an integral role in our team, providing direct care and support.

Basic Qualifications:
Education: Successful completion of a Pennsylvania State approved certification course; or an
approved certification course from another state; or documentation of completion of a Fundamentals of
Nursing Course from an approved School of Nursing. At completion of the fourth month, must hold
current Nursing Assistant certification in Pennsylvania. Current CPR Certification. Successful completion
of the Department of Welfare Medication Administration Course. Successful completion of Diabetic
Education.
Skills: Proficient written and verbal communication skills; ability to work congenially with others,
follow instructions, and show a genuine interest in geriatric nursing. Computer skills.
Experience: Three (3) month prior experience in long term care preferred.
Continuing Education: Must attend 12 hours of related in-service. As mandated 55 Pa. Code Chapter 2600
65e. continue with CPR certification As in 55 Pa. Code Chapter 2600 63b. Must keep Medication Tech
status current. Must complete annual Diabetic education.
General Responsibilities:
- Provides individualized care to residents according to the resident's care plan such as feeding, bathing, dressing, taking vital signs, etc. while maintaining their comfort and dignity.
- Administers prescribed medications and perform simple treatments. Checks medications for expiration dates and reorders medications when supply is low. Assesses resident’s physical, medical and emotional needs. Reports pertinent information to the Administrator/PCF Manager or designee. Responds to inquiries and requests from residents, staff, family members, etc.
- Obtains vital signs for parameters as ordered for medication, Weekly blood pressure and pulse taken, monthly weight and documents on eMAR. Reports abnormal vital sign, increase or decrease in weight to Administrator/PCF Manager or designee.
- Documents pertinent information on the eMAR progress note and informs Administrator/PCF Manager or designee of documentation.
- Provides care that maintains the resident’s skin integrity such as changing incontinent residents, turning, and repositioning residents in bed etc.
- Answer residents’ call bells.
- Takes vital signs as designated by the Administrator/PCF Manager or designee.
- Assists with orienting residents and their families to Phoebe upon admission and to the unit when transfers occur.
- Uses principles of good body mechanics in lifting, moving, and transporting residents.
- Communicates and interacts effectively with the resident.
- Observe changes in the resident's condition or behavior and report these to the Administrator/PCF Manager or designee immediately.
- Collect specimens as requested by licensed personnel.
- Prepares residents for appointments, such as, beauty shop, activities, podiatrist, dentist, church services etc.
- Attends Nursing Assistant meetings.
- Ensures an atmosphere, which allows for the privacy, dignity, and well-being of all residents in a safe, secure environment.
- Maintains certification and meets all requirements as a med tech and reports any errors or needs to the nursing supervisor or administrator immediately. Minimizes errors or asks for additional training as needed in meeting med tech requirements.
- Completes annual compliance and HIPAA training and exhibit behavior as set forth by the Phoebe Ministries Code of Ethics and Business Integrity in the performance of their duties.
- Preforms other duties as assigned.
