Job Overview:

Provides all aspects of care and activities to residents, which must be in accordance with current federal, state and local regulations to ensure the highest degree of quality care will be provided.


Primary Scope of Responsibilities:

  • Performs fundamental duties of resident care (not requiring a licensed nurse), based on the individual needs and plan of care for each resident. Documents necessary information in the appropriate logs and charts each shift.
  • Assesses residents for changes in condition and implements proper interventions. Communicates with physicians, diagnostic services, hospitals, care providers, and families as needed.
  • Helps to maintain the physical environment conducive to the resident’s welfare and best interest.
  • Demonstrates purposeful service excellence through exceeding patient and coworker expectations.
  • Represents the department/hospital as a dedicated, courteous, and responsive employee.
  • Records pertinent documentation in accordance with regulatory requirements, professional standards, and hospital policy.
  • Adheres to federal, state and hospital rules and regulations concerning HIPAA privacy and security.
  • Follows Hospital compliance plan and policies, including the Code of Conduct.
  • Participates in required training for compliance related activities (HIPAA, Compliance, Patient Safety, TJC, etc.)
  • Assists with physical care such as bathing, oral hygiene, dressing, undressing, making beds and other activities of daily living as needed. Washes linen and personal clothing per schedule. Completes hair and nail care per schedule.
  • Administers and orders medications as needed and/or directed. Transcribes medication per physician order. Takes temperature, pulse and blood pressure as assigned. Reviews medication sheets and for accuracy, noting any discrepancies. Obtains physician updates for residents semi-annually and as needed/directed.
  • Assesses residents for changes in condition and implements proper interventions. Notifies family members of change of condition of a resident. Assists with admissions, discharges, and transfers as needed and directed.
  • Assists in providing resident activities. Motivates and prepares resident for activities. Serves meals and snacks to residents.
  • Follows proper fire and safety procedures, infection control and universal precaution policies/procedures, and resident emergency alarm/pendant protocols, reporting any needs for intervention to supervisor.
  • Performs clerical duties, such as maintaining proper documentation, filing, and other tasks as needed and/or directed.
  • Demonstrates responsibility and accountability for own professional decision making and professional growth.
  • Participates and contributes to departmental meetings and activities.

Population Specific Criteria:

  • Completes required training in senior care within 3 days of hire and annually.
  • Completes at least one form of continuing education annually, focusing on senior care issues.


Minimum Qualifications & Requirements:

  • High school graduate preferred
  • Medication Administration certification required or must be completed within 30 days of employment.
  • Experience in health care or human services preferably with seniors.
  • BLS Certification
  • First Aid

Benefits & Compensation:

Hire-in rates of pay are based upon the relevance of years of experience, education and certifications. Our talent acquisition team will help you understand how your direct experience will be considered.

PRN pay range for new hires: $20.43 - $25.99/hr
