Job Description:
This position focuses on the issues affecting the health, safety, welfare and rights of residents in residential care facilities for the elderly. The incumbent will work with all the assigned ombudsmen who cover the RCFE – from the small board and care homes , to the large assistant living facilities. This person will work with the Program Management staff to review and revise the coverage plan of visits to the 58 RCFE in San Francisco. In addition , this person will work with the abuse specialist in responding to and triaging cases of abuse and neglect:The level of acuity allowed in RCFE/ARF has exceeded the capability of some facilities to adequate assess changes and meet increased needs, particularly when meeting needs is mediated by the market place, in terms of increased monthly rates. At the same time RCFE residents are sent to ER and Acute hospitals and the Ombudsman receives mandated reports of abuse/neglect. Linked to those reports is the need to hospitals to discharge back to these neglectful RCFE. The Ombudsman investigating such neglect cases is presented with challenges from two directions: the lack of capabilities to meet increased needs in the RCFE and the pressures from hospitals to return.

Job Description:
While skilled nursing homes are regulated by Federal and State Agencies, the regulatory system for Residential Care Facilities for the Elderly does not capture adequately care and quality of life issues. In addition the Assisted living facilities have grown in number, following a business model with less accountability. The level of acuity allowed in RCFE/ARF has exceeded the capability of some facilities to adequate assess changes and meet increased needs, particularly when meeting needs is mediated by the market place, in terms of increased monthly rates. At the same time RCFE residents are sent to ER and Acute hospitals and the Ombudsman receives mandated reports of abuse/neglect. Linked to those reports is the need to hospitals to discharge back to these neglectful RCFE. The Ombudsman investigating such neglect cases is presented with challenges from two directions: the lack of capabilities to meet increased needs in the RCFE and the pressures from hospitals to return. The San Francisco Long Term Care Ombudsman needs to build capacity to better respond to consumer issues in assistant living. This position will evolve into a team leader for assigned RCFE ombudsmen

Duties:
- Investigate and Resolve cases of abuse and complaints about poor care, rights violations on behalf of residents of residential care homes according to Federal and California statutory guidelines.
- Be trained in the State of California Certification training and Carry out the statutory duties of a Certified Ombudsman in accordance with Federal and California Law
- Attend or organize IDT collaborative meetings to the extent a resident has such a support system to resolve issues of care
- Triage abuse cases with City and County of San Francisco, Adult Protective Services
- Work with the RCFE Ombudsman team to assure periodic Ombudsman attendance in resident councils and coordinate with family councils as they occur in RCFE
- Review weekly received SOC 341 where neglect is alleged in RCFE.
- Conduct and coordinate with the RCFE Team monitoring visit for purpose of identifying levels of care: bed ridden; multiple chronic conditions; multiple medications etc.

Qualifications
- College :BA or Post-Graduate:
- Masters level Graduate Work Preferred. A background in Public Health and health systems optimal. Experience in clinical work would be considered( Ombudsmen do not assess and work within a diagnostic framework)
- Experience in gerontological and disability advocacy, paralegal, or Ombudsman work.
- Must be able to communicate with persons with illness , different types of disability and not prejudge by reaching to solutions.
- Ability to listen to elderly and disabled in a person centered approach.
- Ability for self-directed work
