Job Objective:

To provide extended follow along services to clients who are placed in the community and meet the criteria for extended services. This position reports to the Director of Workforce Development Services who reviews job performance annually.

Essential Job Functions:

  • Acquire and maintain a thorough knowledge of each client assigned and his/her employer.
  • Maintain confidentiality at all times.
  • Develop an individual program plan amendment, as appropriate, based upon the input from the person served, other appropriate personnel, and prior assessment and referral information concerning vocational preferences, education, strengths, abilities and needs.
  • Develop a thorough understanding of the tasks, the physical requirements, and the physical environment in which each job is located.
  • Serve as a liaison between supervisors and clients to facilitate communication and assist with problem-solving.
  • Assist the client and employer to obtain high retention rates for clients in community employment.
  • Serve as an advocate for all clients.
  • Maintain accurate records in the case management system, write reports, and engage in constructive and open communication with the staff.
  • Be aware of outside services and make appropriate referrals based on needs identified in the individual program plan amendment.
  • Assist Workshop Facilitator and Career Center staff as needed.
  • Develop, maintain, and implement a recognition procedure for clients obtaining employment and retaining employment when reaching retention milestones.
  • Facilitate any special projects assigned by the Director of Workforce Development.
  • Complete weekly and monthly retention reports.
  • Create and maintain adequate tracking sheets for clients who are employed.
  • Communicate necessary and important information to all team members.
  • Participate in inter and intra departmental meetings as scheduled.

Specifications:

  • Bachelor's degree or actively pursuing a degree.
  • Excellent written and verbal communication skills.
  • Demonstrated ability to motivate others.
  • Ability to relate to, understand, and be creative in meeting the needs of:
    • individuals with economic disadvantages.
    • employers in the community.
    • referral agencies in the area.
  • Ability to function under pressure, be self-directed, and be responsible for carrying out job related activities with minimal supervision.
  • Ability to meet assigned deadlines.
  • Ability to adjust to a flexible scheduled based on a 40-hour week, organize tasks effectively and prioritize job duties.
  • Ability to communicate orally and hear clearly.
  • Must possess a current driver's license.
