Job Description:
The Nurse Care Coordinator (RN) is proficient in all tasks performed by clinical staff members. Additionally, the Nurse Care Coordinator acts as a role model, trainer, and mentor for clinic staff. The Nurse Care Coordinator works cooperatively with the RN Care Manager and Clinical Practice Supervisor to coordinate and facilitate completion of work required to provide high levels of patient care and service. He/she performs the duties and responsibilities in a manner consistent with the mission, values, and standards of Spectrum Health Services, Inc. (SHS).
Essential Functions:
- Demonstrating excellent practice skills that include leadership, flexibility, priority setting, problem solving, conflict resolution, decision-making, work delegation, organization, and verbal/written communication skills.
- Provides supervision of the LPN(s), including training, disciplinary action, payroll approval, and performance evaluation. Coordinates with the RN Care Manager and Clinical Practice Supervisor to identify and address patient care requests, grievances, quality improvement initiatives and clinical operations process efficiencies.
- Ensures the oversight and compliance with the VFC Program and all immunization programs.
- Ensures that all patient Referral and Provider EMR Buckets are managed effectively, not to exceed daily average of 300 tasks.
- Determines work priorities and assigns work to ensure timely workflow for patient care.
- Manage Clinic Inventory and ensures that all medications, vaccines, crash cart supplies, oxygen tanks have not expired, and emergency preparedness protocols are in place.
- Assists Providers with special tests and procedures within the standard scope of practice.
- Assist in center with patient care as needed, including, but not limited to, rooming patients, medication refills, referrals, and phone calls.
- Coordinates with Providers to determine eligibility of patients for Controlled Substance Case Management. Responsible for ensuring medication refills are completed, patients are compliant with medications, and appropriate follow-up is completed
- Completes medical prior authorization, as needed.
- Responds to patient clinical questions and complaints and ensures that patients are seen appropriately and timely
- Works closely with patients and their caregivers to encourage participation in and promote adherence with their health care treatment plan.
- Acts as a clinical resource to Providers and staff and works in collaboration with Clinical Practice Manager to recommend more efficient clinical tasks and improved workflows.
- Works closely with practice leadership to properly address and resolve patient related incidents, assist with the monitoring of achieving quality metrics, and other clinical initiatives that may arise within the health center.
- Document all services and contacts in a timely manner in the electronic health record system and other tracking systems Records care provided within electronic medical record and completes other assigned reports.
- Support the patients with pre-visit checklist process, including assisting the member in preparing for visit, reviewing health maintenance during their visit, placing orders in advance, and completing visit follow-up work.
- Provides patient outreach by phone and via face to face visits in the health center.
- Works within all state and federal laws and regulations.
- Notifies immediate supervisor of significant problems.
- Makes operational decisions as appropriate in the absence of the Clinical Practice Manager.

Supervisory Responsibilities:
- LPN
Education / Experience
- Licensed and credentialed Registered Nurse in the state of Pennsylvania.
- 3-5 years’ experience in clinical or community resource settings; Care coordination and/or case management experience is desirable.
- Evidence of essential leadership, communication, education, and counseling skills.
- Proficiency in communication technologies (email, cell phone, etc.).
- Highly organized with ability to keep accurate notes and records.
- Experience with health IT systems and reports is desirable.
- Local knowledge about and connections to community health care and social welfare resources is desirable.
