Spectrum Health Companies is a Minnesota-based statewide community health care provider of Assisted Living, Memory Care, Homecare and Independent Living services.

Established Assisted Living and Memory Care community is accepting applications for a RN, Case Manager to join our team.

15-20 hours per week with additional hours based on community and client needs. These hours each week will be to conduct assessments, general follow up with staff, etc.

DUTIES

Selected candidate will provide direct nursing services to clients and through the coordination of all disciplines involved in client care, as assigned by the Manager.

The Case Manager is responsible for resident outcomes, ensuring appropriate care, assessment of resident needs and modifying the care plan as needed.

Selected candidate will communicate effectively with resident/family, physicians, and other disciplines regarding plan of care.

The Case Manager will assist each resident to reach and maintain his or her highest level of performance and independence.

QUALIFICATIONS

Current licensure as a Registered Nurse in the State of Minnesota. Bachelor's Degree in Nursing preferred, but not necessary.

Preferably, a minimum of two years' experience in nursing in health care. Prior supervisory experience preferred.

Position requires candidates to have a high level of proficiency in general computer programs including Microsoft Excel and Word.

Candidates will be subject to a criminal background study and must be able to meet the physical requirements of the position.
