As a member of the Interdisciplinary Team, the RN Case Manager is an experienced Registered Nurse who provides skilled palliative and end of life care in the community setting. The RN Case Manager is responsible for promoting the Hospice philosophy. This individual facilitates the development of a collaborative interdisciplinary plan and ensures the overall coordination, collaboration and medical management of hospice services for patients and their significant others.

- Completed a professional nursing program accredited by the National League for Nursing (prefer completion of a baccalaureate program in nursing).
- One year experience in acute care facility or community health agency, (hospice experience a plus).
- Current RN Licensure and CPR Certification.
- Valid drivers license, auto insurance, reliable transportation and a willingness to use personal vehicle in the course of employment.
- Ability to meet the physical demands of the job (i.e., Push, Pull, Lift/Carry 35 lbs.)
- Availability to rotate weekends.
