Laurel Parc, a beautiful and upscale senior living community, seeks a detail-oriented RN to serve as a Resident Care/Case Manager. This key role will collaborate with our care team, care Directors and residents, mainly in Assisted Living, to ensure a superior level of care and the proper carrying out of service plans. All care areas will rely on this individual for the necessary knowledge and guidance in ensuring the highest level of compliance and quality within our caring community.

This is a full time position, however, some flexibility may be given to total hours per week, schedule, and allows limited ability to work from home after training period.

Responsibilities:

  • Reviews and contributes to service plans.
  • Completes resident Health Evaluations.
  • Reviews incident reports and participates in corrective action plan development.
  • Provides first aid and emergency assistance while working within the community.
  • Acts as a clinical resource to all Resident Services staff.
  • Completes and supervises RN Delegation, as applicable by state law.
  • Provides training programs as requested.

Qualifications:

  • Current, unencumbered Oregon RN license and graduate of accredited nursing program.
  • Professional in appearance and demeanor.
  • Knowledge of applicable regulations pertaining to assisted living or memory care residents a plus.
  • Familiarity with older adult nursing care and the concepts of assisted living or memory care.
  • Outstanding assessment skills.
  • Excellent interpersonal skills.
  • Attention to detail.
  • Ability to perform multiple tasks with frequent interruptions.
  • Outstanding customer service attitude and demeanor.
  • Excellent verbal and written communication skills.
  • Current CPR card required.
