BRIEF DESCRIPTION: : Works collaboratively with physicians to coordinate and screen for the appropriateness of admissions and Continued stays. Reviews alternatives with the physicians for alternate levels of care when the patient does not meet the medical necessity for Inpatient hospitalization. Interacts with the family, patient, and or caregiver and other disciplines to coordinate a safe and acceptable discharge plan. Works collaboratively with a Multidisciplinary team.

QUALIFICATIONS: Licensed and currently registered to practice as a professional nurse in New York State required. B.S. degree preferred. Previous case management required, Discharge planning experience required. Two (2) years of clinical acute care nursing experience as an RN required. Demonstrated ability to utilize/apply the general and specialized principals, practices, techniques and methods of Utilization review/management, discharge planning or case management. PRI and Screen Training Working knowledge of regulatory requirements and standards Demonstrated ability to utilize written and verbal communication, interpersonal, critical thinking and problem-solving skills. MDS experience preferred. Computer skills required.
