RN Case Manager for Homecare

Looking for an RN who is motivated and positive.
We are seeking an RN Case Manager to join our team! We're a small town company that loves all our staff and our nurses. You will be responsible for opening home-care cases and coordinating with the office staff. This position travels within the surrounding 5 county area. Drive time is paid.
Responsibilities:
- Coordinate with lead nurse to admit and open cases in the homes of clients.
- Create and review care plans.
- Perform followup visits.
- Oversee unlicensed personnel.
- Provide training and sign off on competency.
Qualifications:
- Familiarity with medical software and equipment
- Ability to build rapport with patients
- Strong problem solving and critical thinking skills
- Ability to thrive in a fast-paced environment
- Communication skills.
- Coordinating schedules.
Job Type: Part-time
Pay: $30.00 - $33.00 per hour
