RN Case Manager (Partial Work From Home / Flexible Schedule) - RESPONSIBILITIES

• Case Management - Take client calls, Resolve client concerns, perform conflict resolution, make physician referrals, call in hotlines when needed, facilitate communication between parties when needed, correspond with state regarding client and care plans
• Client Advocacy - Help clients take advantage of services available to them, communicate with different agencies to provide extra support, help clients sign up or reactivate their Medicaid services
• Client Visits - Complete initial client assessments, general health assessments, medication management, PRN /checkup visits, APC visits, and supervisory visits
• Nurse Paperwork Auditing - Audit all nursing paperwork for all clients for accuracy, completeness, and timeliness
• Compliance - Ensure all nursing policies and procedures are followed by nurses, assist with state audits and inspections, submit all required reports
• Training - Train caregivers on Advanced Personal Care tasks and other specialized training, especially related to client IADLs and safety.
• General Office Tasks - assist fellow office staff, maintain visit documentation and client records in computer systems

RN Case Manager (Partial Work From Home / Flexible Schedule) - QUALIFICATIONS

• Must have a current license in good standing to practice as a Registered Nurse (RN) in the State of Missouri.
• At Least 3 years of experience required as a nurse.
• At Least 1 year of case management experience.
• Valid Driver’s License and reliable transportation and agency required liability insurance to complete home visits.
• Successful completion of a criminal background check and drug screening.
• Effective communication skills and ability to interact and build relationships with clients, clients' families, other healthcare professionals, and office staff.
• Basic computer literacy.
• Impeccable documentation and paperwork.
• Experience in the home care field is a plus.
• Supervisory experience is a plus.

• Salary: $80,000.00 + Depending on Experience
• PTO: 2 Weeks per year
• 1 Week paid sick leave
• $2,000.00 Sign on Bonus
