Your role at PFH:
As a RPG Peer Mentor, you will provide recovery-focused case management and rehabilitation support to grant families living with substance use concerns, mental/emotional disorders and environmental stressors.
You will:
- Mentor, advocate, role model and coach OP and IOP clients through the provision of individual educational sessions and facilitation of group education under the oversight of Clinical Supervisor. (Residential clients are not eligible for Peer Services).
- Represent Preferred Family Healthcare (PFH) Treatment Services and client served in a positive manner to clients, staff, community agencies, and community members.
- Advocate for clients within multiple systems.
- Treat all persons served and their families with dignity and respect.
- Assist new families and those transitioning from a crisis setting by offering hope, assistance in navigating through services and on-going support.
- Contact families as indicated by individual treatment plan or as necessary to determine needed resources and assistance while providing support and encouragement in the recovery process.
- Provide referral, linkage, advocacy and monitoring for families. Provide transportation to families as required to various resources in the community.
- Provide emotional, informational, and instructional support while assisting clients in feeling connected and help clients develop recovery activities and skills.
- Promote the clients in an effort to live a healthy, productive, sober lifestyle during and after they leave the facility.
- Assist participants in developing personal treatment plans in order to be more actively involved in their own treatment episode.
- Provide clients with information and referrals to appropriate outside services, and other community based support groups in conjunction with treatment team and under the oversight of the Clinical Supervisor.
- Document all encounters with the client using facility approved methods.
We think you'd be great for this role if you have:
- Completed the required 6 contact hours of a KCPM/AAPS approved training, which includes ethics and confidentiality.
- High School/GED.
- CRPR certification from the Missouri Credentialing Board.
- Peer Recovery Mentor must have at least 2 years of sustained sobriety AND a positive relationship with the recovery community.
