Our Senior Services Line Support provides telehealth support for NARA’s older population, our elders, as well as resource referral, care coordination, patient support in maintaining housing, basic needs, and independent living skills, and crisis intervention. You will work as a part of the mental health team and in close collaboration with the Elders Team, NARA’s Housing Services, medical services, and other NARA programs. The ideal candidate will have personal and/or professional experience supporting elders in their own homes or in a setting where elder care is provided (adult foster care, nursing facility, or assisted living setting, for example).

Essential Job Duties:

  • Provide phone and/or video support to elders in the local community.
  • Work with Elder Navigator to refer to elders to resources and support engagement in medical, dental, or behavioral health.
  • Assess suicide risk for callers and take necessary steps to ensure safety.
  • Respond to other emergency situations by engaging rapid response systems as needed, including 911.
  • With the Crisis Care Manager, assess needs for crisis line versus warm line utilization and develop plans to respond to community needs.
  • Monitor food and housing security.
  • Provide community support and resources to elders living with serious mental disorders that significantly affect their functioning in the community setting, including referrals to emergency or urgent medical settings or primary care providers.
  • Refer to appropriate mental health or addiction services as needed that address elders’ needs.
  • Assist with connection to cultural resources, activities, and events. Work with Elders’ Team to assist with transportation or other needs.
  • Provide a warm hand off for elder who are being referred out to resources.
  • Maintain logs and clinical records documenting support calls, follow-up, and outcomes.
  • Coordination of care – actively initiate and maintain communication and collaboration with all members of elder’s support system and service team as permitted by the elder.
  • Maintain appropriate documentation as required by agency policy and OARS, learn elder specific documentation in external systems as needed and approved.
  • Connect with elder and disabled services public care delivery system, as needed.
  • Participate in multi-disciplinary team meetings to coordinate care, including older adult behavioral health services and the older adult behavioral health initiative trainings.

Qualifications:

  • Credentialing as a QMHA or peer support specialist through MHACBO or willingness to become certified upon hire is required.
  • Previous experience with older adults and/or Native American/Alaska Native populations strongly preferred.
  • Ability to be credentialed as a peer support provider or recovery mentor would be considered an asset.
  • Desire to serve elders in a supportive capacity, previous experience preferred.
  • Additional training around senior services, culture, and knowledge of community resources and entitlement programs preferred.
  • Understanding of common health and behavioral health issues for older patients.
  • Ability to engage and interact positively with elders to promote strengths and improve health and wellbeing.
  • Initiative to communicate effectively in coordinating care.
  • Excellent communication skills.
  • Knowledge and skill in working in a multi-cultural environment.
  • Good understanding of professional role and boundaries.

Job Type: Full-time

Pay: $22.00 - $24.00 per hour
