What will I be doing?

We are looking for a Service Coordinator and help us advance our mission. The Service Coordinator is responsible for providing support to people receiving services from Trivium Life Services. They are also responsible for ensuring each person served is provided the opportunity to enhance their life through the process of a person-centered plan. Come be a part of this journey with us.

A quick highlight of some of the things our coordinators do:

  • Develop, monitor, and support an Individual Support Plan in collaboration with person served, to achieve goals.
  • Write, coordinate, & ensure the implementation of behavioral plans.
  • Perform persons served intake, discharge, termination, and follow-up tasks.
  • Work with Finance Dept. to secure and process client authorization for both new and existing clients.
  • Maintain accurate and timely client information in Therap.
  • Ensure adherence to persons' served medication management plans.
  • Abide by all HIPAA regulations and employ safeguards as directed by HIPAA laws and the Trivium Compliance Officer.
  • Update client data systems on a regular basis to ensure accuracy for goal planning and compliance.
  • Participate in meetings for coordinating with the person served and their interdisciplinary team.
  • Facilitate family member support in achieving and maintaining the assigned individual's goals.

What do I need?

  • High School Diploma or equivalent.
  • Two years' experience in working with people with disabilities in a service delivery related field.
  • Must obtain Medication Manager certification within six months of employment.
  • Must be proficient in Therap or utilized EHR within six months.
  • Associate degree from an accredited college or demonstration of a minimum of 60 college credit hours preferred
