SERVICE COORDINATOR, Youth Service Coordination Project

POSITION DESCRIPTION:

Urban Youth Alliance International, Inc. (UYAI), the parent organization, is a community and faith-based agency dedicated to promoting the leadership and self-sufficiency of urban families. In 1999, UYAI established BronxConnect to address the youth incarceration epidemic impacting communities of color. BronxConnect has since expanded to a wide array of services and initiatives, including alternative youth justice, employment, reentry, advocacy, therapeutics, and Cure Violence interventions. We current serve our communities via ManhattanConnect, StatenIslandConnect, PhillyConnect, InCommunity Reentry and Release the Grip (Cure Violence.)

There will be two Service Coordinators (one placed in the Bronx and one placed in Staten Island) across the Project who will support the coordination of services for their assigned location. Service Coordinators will regularly interface with participating youth and their families, local city agency caseworkers, and community-based partners.

The Office of Neighborhood Safety (ONS) within the Mayor’s Office of Criminal Justice (“MOCJ”) is launching a Youth Services Coordination project in collaboration with three community-based organizations, who will work as lead service providers based on geography. The Youth Services Coordination project will be the central coordinating mechanism for delivering voluntary mentorship and crisis intervention, and connection to city services and community supports to one-hundred youth ages 12-15 and 16-19 years old and their families living across select neighborhoods in four NYC boroughs (Brooklyn, the Bronx, Manhattan, and Staten Island).

Southside United/Los Sures and ONS will co-design the pilot project with Urban Youth Alliance International (serving the Bronx and Staten Island) and Living Redemption Youth Opportunity Hub (serving Manhattan). Together, this group of organizations will develop a scalable model and will work in partnership with city agencies and community-based organizations at a local and system level to streamline services for youth and their families who have multi-system involvement. Los Sures, Urban Youth Alliance, and Living Redemption will ensure service delivery to eligible youth and their families on a voluntary basis while leveling up insights to MOCJ and other senior agency leadership to improve the City’s service delivery efforts to high-risk youth and families, who are also experiencing multi-system involvement (e.g.: simultaneous child welfare and criminal justice system involvement).

RESPONSIBILITIES:

Other duties of this role will include but are not limited to the following:

(1) Leads youth case management and coordination of youth service plans;

(2) Supports social worker, when appropriate, with helping families access appropriate resources and services in the right sequence and in a streamlined manner;

(3) Troubleshoots severe or chronic youth service issues with social worker's guidance and support;

(4) Leads, in partnership with the Navigator, youth, community, partner and referral outreach;

(5) Secures community resources to support the development and facilitation of group work and youth-led community projects, as necessary; and,

(6) Updates case management system regularly and helps Social Worker prepare insights for reports to Project leadership, Los Sures, MOCJ and other city agency leadership.

(7) Other tasks as requested by the Project Director or Site Coordinator and necessary for Project implementation.

QUALIFICATIONS:

Person(s) in this role should have a Bachelors’ degree and 4 years experience with case coordination and evidence-based youth development models, experience in youth and family direct service work, youth development work, crisis intervention, or community engagement work or the equivalent. The Service Coordinators will have strong verbal and written communication skills and be highly competent, empathetic, and culturally sensitive to the issues and social context of participating youth and families. Experience in community outreach efforts and/or community-centered programming a must. Person should have the ability to creatively solve problems, address obstacles, work independently and in conjunction with team.

SCHEDULE: M–F, 10am-6pm or 11am-7pm (some nights and weekends.) Flexibility required.

Job Type: Full-time

Pay: $55,000.00 - $65,000.00 per year
