Overview:
Under the supervision of the RN (registered nurse) Care Manager Supervisor, the Sexual Health Patient Navigator will provide patient support and navigation services to CommUnityCare (CUC) patients that are HIV negative but are vulnerable to HIV acquisition and/or who may have barriers to accessing and adhering to HIV prevention with PrEP (pre-exposure prophylaxis) care. This role will support the development of systems and infrastructure to increase PrEP services within CommUnityCare. These services will be provided either in-person or via phone at one of our clinic locations.
Additionally, this Patient Navigator will be point of contact for patients identified as living with hepatitis c virus (HCV) infection to provide linkage to providers with the goal of treatment and cure. The navigator will work closely with the Sexual Health Program Coordinator to help improve HCV workflow within CUC, from initial diagnosis to viral suppression. Using data review and monitoring skills, the Patient Navigator will assist healthcare teams in streamlining the linkage to care process. This role will work closely with Prescription Assistance Program (PAP) specialists to ensure timely access to medication for patients living with HCV or seeking HIV prevention with PrEP. This position will be funded through a Health Resources & Services Administration (HRSA) grant as part of the Ending the HIV Epidemic (EHE) initiative, and through a Gilead FOCUS grant as part of HCV linkage to care.
Responsibilities:
Essential Duties (at least 5 that are non-negotiable duties and are absolutely pertinent to successfully completing the job without accommodations):

Primary Accountabilities:

  • Coordinate, in conjunction with the Care Manager Supervisor and other CUC HIV and Sexual Health leadership to develop and support a navigation program for individuals to access PrEP and HCV care.
  • Function as the primary point of contact for CommUnityCare PrEP and HCV services, providing in-person services for people seen at one or more of our clinic locations and work with other team members to support CUC providers and clinical staff who are providing PrEP services in other clinics.
  • Provide benefit screening and enrollment assistance to improve access and adherence to PrEP services.
  • Provide service linkage to address other psychosocial needs as appropriate; make referrals to behavioral health counselors at CUC for more complex psychosocial intervention as indicated.
  • Reduce barriers to obtaining clinical PrEP and HCV care/services.
  • Assist the clinical team with individualized risk-reduction counseling and support as well as adherence strategies for targeted outreach interventions to optimize follow-up.
  • Assist in the development of tools that support best practices in PrEP and HCV care.
  • Assist in the development of materials promoting PrEP and HCV resources and services.
  • Promote the PrEP and HCV Navigation and clinical services within CUC and with community providers and agencies.
  • Provide focused PrEP outreach activities with vulnerable communities.
  • Perform HIV prevention rapid testing at outreach events to support increased community testing and place referrals as needed to appropriate service lines for continued care.
  • Collaborate with key personnel at CUC clinics and other service/health-related organizations to help develop infrastructure to provide PrEP and HCV care in additional clinics (primary care).
  • Participate in quality initiatives as requested.
  • Acquire and maintain strong knowledge of local, state, and national PrEP-related resources and services to assist patients in accessing PrEP care.
  • Develop and maintain a comprehensive patient resource list, sharing with patients, internal and external stakeholders.
  • Coordinate and present at educational events locally and regionally.
  • Perform other duties as needed.

Knowledge/Skills/Abilities:

  • High level of skill at building relationships and providing excellent customer service.
  • High degree of knowledge and competency in HIV prevention and treatment.
  • High level of problem-solving skills to better serve patients and staff.
  • Strong attention to detail and accuracy.
  • Excellent verbal and written communication skills.
  • Demonstrated knowledge of community resources available and how to access resources for the benefit of
clients.
  • Demonstrated success in collaborating with other healthcare providers for patient-centered care.
Qualifications:
MINIMUM EDUCATION: Graduation from high school

PREFERRED EDUCATION:
Bachelor’s Degree in social work; counseling; nursing, health education or related field

MINIMUM EXPERIENCE:
Two (2) year of experience working within a community health setting in a health care role or a community based human service agency as a Community Health Worker (CHW), health educator, community outreach worker, case manager or community organizer.

PREFERRED EXPERIENCE:
Experience with HIV care and/or prevention, prescription assistance and experience with navigating prescription access. Experience working with gender- and sexual-diverse populations. Bilingual (English / Spanish); Licensed Social Worke
