Martha and Mary, in Poulsbo, WA is currently seeking a Full Time (30+hrs/week -full time) dependable Social Work Clinician to join our team. The Clinician will provide counseling with residents and family as appropriate and requested by residents, family or staff.

Wage:

The wage scale offered for this position is from $24/hr. and up to $26.64/hr. depending on relatable experience as determined by the supervisor and HR.

Duties:

  • Administer mental status testing and depression scale screens to all new residents and re-evaluate as indicated.
  • Attend multi-disciplinary resident care conferences as resident caseload indicates.
  • Provide resident and family assessment information via background social history as the psycho-social database for comprehensive care planning and treatment.
  • Develop, implement and provide facilitation and/or coordination of therapeutic resident support groups.
  • Assist in discharge planning of residents to a more appropriate care setting.
  • Assist in education / training sessions to staff, residents, and families via in-service training, family meetings, resident groups, etc.
  • Keeps records and chart social service interventions.
  • May be required to assist in the dining rooms as appropriate to enhance the resident dining experience.
  • Reliable attendance for scheduled hours and the willingness to be flexible in covering additional hours, including weekends, holidays, and other times.
  • Ability to problem-solve and handle unexpected/unpredictable circumstances. Ability to take initiative

Required Experience:

  • Bachelors’ Degree in psychology, counseling, social work or other related clinical degree.
  • Minimum of 1 year of supervised social work experience in a health care setting
  • Knowledge of procedures and policies related to resident abuse and neglect.
  • Genuine interest in caring for the elderly and long term care of residents.
  • Strong clinical knowledge.
  • Competent documentation skills
  • Superior communication skills with internal and external stakeholders and be able to communicate clearly and effectively both verbally and in writing.
