Responsibilities include:

• Ensuring resident adjustment to rehab or long-term care by performing psychological screening, initial care planning, introduction to services and procedures and completing all necessary paperwork.
• Participating in admission process, grievances, MDS assessments, counseling, IDCP meeting, d/c planning.
• Following policy and procedures, federal and state compliance with familiarity within CMS regulations.
• Long Term Care Experience

Qualifications:

• Must be a Licensed Social Worker in good standing.
• Completion of secondary education at an approved school of Social Work.
• Previous experience in supervisory capacity preferred.
• Knowledge of a specialized field such as geriatric, adult social work, or long term
• Must have the ability to relate professionally and work cooperatively with residents, families, employees, and other stakeholders at all levels of the organization and community to ensure continuity of care.

Job Types: Full-time, Part-time

Pay: $27.00 - $33.00 per hour
