What’s a Dementia Hub Social Worker?

Oakwood Creative Care, a non profit specializing in "day clubs" has developed a new program called the Dementia Hub that will bring support, education, and community together to create a place that supports seniors and their families. This new position will provide the opportunity to leave a lasting imprint as part of a dynamic team of experts that will work together to combine their expertise, interests, and passions to make a difference. At OCC, we are a passionate, creative, out-of-the-box thinkers, who are finding ways to bridge the gap in what Medicare will pay for and what our community actually needs. We don’t have all the answers, but we are obsessed with figuring it out. We think that when we lead with giving purpose, meaning, respect, and fun regardless of a fading memory we can make a tremendous impact. We are here for the challenge and love what we are doing. Our team is driving the conversation in removing the stigma of dementia care and lack of resources that are easily assessable to our community. This new program is pioneering an entirely new way to bring together families and people living with dementia giving them a wrap around opportunity to reduce isolation, relieve care partner burnout, and improve quality of life. The team is made up of an Occupational Therapist, Nurse, Hub Coordinator, and you (the Social Worker).

Responsibilities include:

• Conduct support groups at Memory Café’s.
• Host educational classes at the Dementia Hub and around the community.
• Collaborate in the development of creating fun community events such as ice cream socials, paint parties, happy hours, drum circles, etc.
• Guide clients and their families through the journey of dementia while offering support and resources
• Serve as a pillar of advocacy for families
