Sunshine Care is a geriatric medical practice serving the LTC/AL population. If you live in these areas please apply. Case manager experience required.
Job will be in Enid at 2 locations.
Must be fully vaccinated due to the nature of the position.
Responsibilities
- Help patients achieve wellness and autonomy
- Facilitate multiple care aspects (case coordination, information sharing, etc.)
- Help patients by providing information to make informed decisions
- Develop effective working relations and cooperate with facility throughout the entire process
- Go the extra mile and interact with patients to keep track of their progress and to ensure satisfaction
- Record information and complete accurately all necessary forms
- Assess and address motivational and psycho-social issues
- Adhere to professional standards as outlined by protocols, rules and regulations
Skills
- Proven working experience in case management, including as a nurse, medical, mental health care manager or a related job
- Excellent knowledge of case management principles and healthcare management
- Previous experience with psychological aspects of care
- Effective communication skills
- Excellent organizational and time management skills
- Familiarity with professional and technical emerging knowledge
- Problem solving skills and ability to multi-task
- Compassionate with teamwork skills
- BS degree in related healthcare field or enrolled in a bachelors program in case management or social work required.
Job Type: Full-time
Pay: $17.50 per hour
