Sunshine Care is a geriatric medical practice serving the LTC/AL population. If you live in these areas please apply. Case manager experience required.

Job will be in Enid at 2 locations.
Must be fully vaccinated due to the nature of the position.

Responsibilities

• Help patients achieve wellness and autonomy
• Facilitate multiple care aspects (case coordination, information sharing, etc.)
• Help patients by providing information to make informed decisions
• Develop effective working relations and cooperate with facility throughout the entire process
• Go the extra mile and interact with patients to keep track of their progress and to ensure satisfaction
• Record information and complete accurately all necessary forms
• Assess and address motivational and psycho-social issues
• Adhere to professional standards as outlined by protocols, rules and regulations

Skills

• Proven working experience in case management, including as a nurse, medical, mental health care manager or a related job
• Excellent knowledge of case management principles and healthcare management
• Previous experience with psychological aspects of care
• Effective communication skills
• Excellent organizational and time management skills
• Familiarity with professional and technical emerging knowledge
• Problem solving skills and ability to multi-task
• Compassionate with teamwork skills
• BS degree in related healthcare field or enrolled in a bachelors program in case management or social work required.

Job Type: Full-time

Pay: $17.50 per hour
