The Position: Social Worker ( BSW)
The Social Worker is responsible to assist family in onboarding patients by assessing their home care needs and verifying financial situtation.
What we ask for:
- Bachelors Degree in Social Work required (BSW)
- Experience with case management
- Bilingual a plus
- Familiarity with Medicare, Medicaid and community resource programs.
- MUST be fully vaccinated against COVID 19
