Glendive Medical Center is seeking a full time Social Worker for Hospice, with additional hours in other departments. The Social Worker is responsible for providing social services to patients who are receiving hospice services and are in need of assistance with social and emotional problems related to their medical conditions. This position is responsible for aspects of clinical care including assessments and Plans of Care, bereavement services coordination, and hospice volunteer coordination to fit the patient's needs. This position is responsible for complying with policies, procedures, and standards of care.

• Bachelor's degree in social work, human development, or related field
• MSW/LCSW Preferred
• One year experience in health care setting preferred.
• Valid Driver's License is required
• Good visual, hearing, keyboarding, and communication abilities required
