Overview:
Location: Ft. Bragg, NC
Pay rate: Up to $70,000/Year
Work Schedule: Shall be required to work 40 hours per week. The normal hours of work are Monday through Friday, between the hours of 6:00 am to 11:00 pm (excluding federal holidays). Typical hours 7:00 am to 4:00 pm.

Responsibilities:
  • A fully qualified typist with a minimum of 40 WPM is required.
  • Ability to plan, organize, schedule, and coordinate activities such as appointments, meetings, and suspense timelines.
  • Proficient in Microsoft Office software. Able to use various programs, such as Word, Excel, Access, PowerPoint, and Project to generate a range of products (i.e., tables of contents, import graphs or databases, create glossaries, align multiple columns, print document, identify files or make other notations at the top or bottom of each page, and create form letters and automatically merging these with mailing lists).

  • Must have knowledge and skills to effectively apply SNC functions: a) Assessment: Identification of patients that need special medical and education needs/management; comprehensive collection of patient information and medical status; and continued evaluation of an established plan of care; b) Planning: Collaboration with the patient, family/caregiver, primary provider and other members of the health care team for developing an effective plan of care; c) Facilitation: Care coordination and communication among all involved parties; d) Advocacy: Support for the patient and family/caregivers to ensure identified education and appropriate, timely care coordination is received.

  • Serve as the Medical Social Worker Special Needs Coordinator (SNC) for the medical treatment facility (HRP).

  • Tracks admissions and discharges of all Mental Health (MH) patients.

  • Provides high interest case management/monitoring for MH.

  • Coordinates psychiatric medication refills.

  • Arranges and follows internal/external referrals for MH.

  • Monitors no-shows and follow-up for MH patients.

  • Tracks patients during hospitalization, intensive outpatient and partial hospitalization.

  • Coordinates and consolidates internal/external MH care.

  • Oversees transfer of patients and records upon permanent change of station (PCS).

  • Identifies local community/network resources.

  • Documents case management in patient charts.

  • Collects and tracks documentation of external MH care for patients.

  • Participates in MCCC discussions whenever they occur.

  • Collaborates with ARC medical personnel in all matters dealing with ARC personnel.


Qualifications:

Education: Masters of Social Work (MSW) degree from an accredited graduate school of social work.

Experience: A minimum of two years full-time experience in care coordination, discharge planning or case management in a medical setting within the last four
Certification: Basic Life Support (BLS).
