SPECIALIST, OUTREACH (STAR HEALTH)
Job description
- Collaborate with NYC Department of Health and Mental Hygiene (DOHMH) to develop outreach and engagement protocols to identify and engage PWH who are out of care using multiple internal and external data sources.
- Provide diligent outreach to locate hard-to-reach clients and or clients lost to service. This includes travel to conduct home visits, hospital visits, or visits in other settings to follow up on care. Develop working relationships with health care facilities and community organizations in Brooklyn neighborhoods which have the highest proportions of PWH who are out of care have the highest levels of food insecurity and poverty and lowest levels of education.
- Serve as the primary liaison with other health care facilities and community organizations to ensure out-of-care PWH are linked to care at STAR-Brookdale or other health care facilities near their homes.
- Develop system of follow-up with patients and providers to check on care status.
- Provide individual and group education to the community and partnering agencies regarding the importance of 1) knowing one’s HIV status; 2) if HIV+, immediately engaging in care and initiating ARVs to reduce viral load to an undetectable level; 3) treatment as prevention (Undetectable = Untransmissable) to protect sexual partners against HIV transmission; and 4) the availability of pre-exposure (PrEP) and post-exposure prophylaxis (PEP) to prevent HIV transmission.
- Provide warm hand-off to the Patient Navigator at STAR-Brookdale or key person at other care sites to ensure each patient is linked to their first appointment
- Provide leadership to STAR-Brookdale participation in community-based events to provide rapid HIV testing and community education on prevention and care of HIV.
- Participate in the development and dissemination of HIV care and prevention social media campaigns.
- Document all outreach encounters in eShare database, as appropriate and document patient-specific encounters in the electronic health record (EHR).
- All other duties as requested.
Education:
- High school diploma/equivalent; some college preferred.
Experience:
- Minimum two (2) years’ experience providing health education or outreach in a health care or social service agency.
- Experience with underserved populations such as people living with HIV/HCV, LGBTQ, and those with mental health and substance use disorders strongly preferred.
Knowledge and Skills:
- Ability to communicate in a warm, caring manner and to motivate others.
- Knowledge of community resources and experience in traveling within the community.
- Ability to travel within Brooklyn via public transportation (with occasional travel to other boroughs).
- Empathy towards and a desire to work with persons living with HIV/AIDS. Ability to be a team player, understand and carryout directions, and to communicate effectively with supervisors.
- This position affords the ability to outreach potential candidates using telehealth services however, fieldwork is also required.
- Training in Motivational Interviewing, ARTAS, and Trauma Informed Care required, but may be completed upon hire.
- Experience providing point-of-care HIV testing preferred.
- Bilingual English/Spanish or English/Haitian Creole preferred.
- Experience with eSHARE data entry systems and Epic reporting preferred.
- Ability to fully utilize electronic record systems, equipment and other healthcare, billing and/or word processing systems relevant to this position.
- Position requires prolonged periods of sitting and use of a computer and screen viewing throughout the working day.
- Position requires prolonged periods of sitting, standing, reaching, and walking throughout the working day.
- Position will be required to stoop, bend, lift, and carry items weighing up to 25 pounds.
