The Staff Nurse provides oversight and supervision to home service workers and CNAs, guides patient care and service delivery through assessment, management and oversight, working with clients and/or their responsible parties.

** 12 hour shifts 8:00am to 8:00pm, mostly week days, but some weekends possible

Essential Duties and Responsibilities

Clinical Evaluations and Care Plan Development

  • Assess and evaluate the clinical/wellness needs of clients.
  • Evalute the safety of the home environment for both the client and the home service worker or home health aide.
  • Evaluate and recommend tele-technology services appropriate for the client to assist in monitoring health and communication to healthcare providers.
  • Create a plan of care, securing physician approval if necessary, to meet the needs of the client.

Supervison of Staff

  • Orient home service worker/home health aide to the client and the care plan.
  • Establish protocols for care and documentation standards
  • Monitor and support staff in deliviery of services.
  • Conduct site visits periodically to review documentation and client care plan needs
  • Chart progress notes in factual manner, including care administered and client responses.
  • Review documentation and advise staff as needed.
  • Evaluate staff performance on a client basis, sharing information with Director of Operations.

Clinical Services

  • For home nursing clients, perform the functions of medication administration and/or filling medication boxes
  • Assist clients in communication with healthcare providers
  • Communicate with physicians and/or families regarding needs and changing conditions
  • Review and report any discrepancies noted concerning physician orders, dietary changes, charting issues, etc.
  • Conduct wellness clinic at Montgomery Place under their established program.

General Responsibilities

  • Adhere to the policies and procedures of the business with the highest practical standards
  • Participates in and maintains current on all training and education required by the company
  • Communicate effectively with all members of the staff and business partners
  • Provide feedback to others for continuous quality improvement of the entire organization

  • Escalate issues when there is appearance that business standards of quality are not present
  • Comply with all rules and regulations of the organization, including HIPAA and safety
  • Perform other duties as assigned
