Remind Health Group is seeking a full-time Targeted Case Manager capable of serving a full 25-person case load. In exchange for these services, Remind is offering a generous monthly salary and additional pay options monthly to this valuable teammate. The Targeted Case Manager is responsible for care coordination during the primary and structured recovery process while under ReMind Health care. Targeted Case Management provides client advocacy while linking and referring clients to community resources to promote independence, growth, and support that will enable individuals to maintain long-term sobriety.
Job Posting:
· Maintain confidentiality of records relating to patients’ treatment
· Prepare and maintain all required treatment records and reports.
· Collect information about clients through interviews, observation, and tests
· Obtain prior authorizations to avoid lapse of insurance coverage when required
· Maintain and complete TCM compliance log daily
· Function as patient advocate to coordinate required services or to resolve emergency problems in crisis situations
· Assist clients with transportation scheduling
· Continuously assess and evaluate referral resources to determine their appropriateness
· Arrange referrals to other professionals, agencies, and community at large to identify service gaps, expand community resources, and help address unmet needs
· Follow up on referral outcomes, exchange relevant information to referral agencies in a manner consistent with confidentiality regulations and professional standards of care
· Assist client in developing informal support systems
· Complete weekly Supervision and maintain documentation
· Performs other duties as assigned by management
Requirements:
· A Bachelor of Arts or behavioral science degree and 1 year of full-time employment experience collaborating with individuals with SMI, SED, or SUD and co-occurring chronic or complex physical health condition, OR
· Master's degree in behavioral health science and two years of full-time experience providing service coordination or linking/referring for community-based services for individuals with SMI, SED, or SUD and co-occurring physical or behavioral health disorders.
· Must have TCM training certificates (Core, SUD, and SMI).
· Computer literate and ability to communicate clearly, both in writing and verbally with clients, administration, the treatment team, and family members
· Must pass background check
· Must be absent from the Federal Exclusion List

Pay: $32,400.00 - $45,000.00 per year
