Job Title: Care Management Team Leader
Location: Mt Vernon, NY
Salary 65K yearly
Care Management Team Leader (CM TL) will co-lead the team under the direct supervision of the CM Coordinator (and the Director). They will support and motivate team members, monitor performance, and provide effective guidance. The CM TL must be able to act proactively to ensure smooth team operations and effective collaboration. They will lead by setting a good example and engage the team to achieve goals.
The CM TL will assist CM leadership in developing a training plan and training new hires, reporting on team performance, enforcing all policies, procedures, protocols, guidance, and regulations.
Responsibilities
- Monitor team performance,
- Track and report on metrics (i.e. ensure documentation in GSI recording in IMA, and follow-up of hospitalizations, ER visits, 7 and 30 day follow up post discharge)
- Maintain team billing log (update daily) and monitor individual care manager QA logs
- Track enrollment, outreach, disenrolled, and pending statuses, dates, and programs for all members
- Eligibility documentation

- Consents
- Comprehensive Assessment completion
- Face to Face visits
- Other contacts
- HML completion
- Care Plan completion and signed
- Notices of enrollment and Disenrollment
- Medicaid recertification due date

- HARP status
- Proficient on all care management platforms (i.e. use dashboards and pull reports)
- GSI
- BTQ
- MAPP
- HealtheConnections
- EPaces / EMedNY
- PSYCKES
- The lead for all HARP clients
- Complete brief assessments and plans of care
- Communicate with MCO
- Receive level of service determination and connect client to HCBS provide
- Create an inspiring team environment oriented to trust, open communication, creative thinking, and cohesive team effort
- Set clear team goals
- Facilitate problem solving and collaboration
- Delegate tasks and set deadlines
- Conduct weekly team meetings or daily huddles
- Oversee day to day operation of the team (appointments, vehicles, coverage)
- Motivate team members, encourage constant improvement
- Discover training needs and provide coaching
- Listen to team members’ feedback and assist in resolving any issues or conflicts
- Suggest and organize team building activities
- Work with the CM Coordinator to develop a multifaceted rewards system that recognizes and celebrates top performance (productivity, billable services, documentation requirements)
- Carry a caseload of 35 clients

Education/Experience
- Bachelor’s Degree in related field (health / human services, medically underserved)
- At least 3 years experience in care management
- Supervisory experience preferred
