Job description

• Monitor ADTs from hospitals for ACO patients presenting in the Emergency Rooms. Assess situation to determine if intervention or support via consultation is required.
• Consult with Hospitalist or other clinicians to determine whether the patient could stay in Observation overnight, instead of being admitted to the hospital.
• Contact and/or follow up with the respective ER clinician as needed.
• Provide ongoing updates as necessary depending on the patient condition. Update case status in EMR system.
• Maintain open communications and develop relationships with hospitals to work toward common goals of reduction of costs and continuity of care.
• Ensure our patients are properly identified at the point of service throughout the care continuum.
• And other duties as assigned

Working Conditions: Standard office conditions.

Education Required:

• Bachelor's Degree (RN)

Education Desired:

• Master's Degree (Nurse Practitioner)

Licensing Required:

• Nurse Practitioner or RN

Experience Required:

• Significant experience working in an ER Department is required

Knowledge, Skills, & Abilities Required:

• Excellent analysis and problem-solving skills.
• Ability to use data to measure progress and achieve outcomes.
• Strong business communication skills to resolve issues and communicate with internal and external parties
