General Description

The Transitions Coordinator will be responsible for planning and facilitating the transition of students with intellectual and developmental disabilities for life after high school. In addition, the Coordinator works as a collaborative team member with the leadership team to implement initiatives to expand creative opportunities for transitioning students.

Essential Responsibilities

  • Coordinate transition of students with disabilities ages 17-21.
  • Participate in meetings and contribute to the planning and preparation for transition to post high school life.
  • Provide consultation to students, families and staff.
  • Maintain working and updated knowledge of community resources. (vocational, respite, funding, residential)
  • Maintain a collaborative relationship with ORS (Office of Rehabilitative Services)
  • Understand and oversee monthly billing procedures and processing.
  • Meet with supervisor monthly to discuss transitions program progress, concerns and highlights.
  • Coordinate and participate in all Professional Meetings and Staff Meetings.
  • Support the coordinator by providing programmatic feedback on the Transitions program assisting with: onboarding new students, community site development and program enhancements.
  • Provide leadership to West Bay Transitions staff and students.
  • Maintain regular communication with staff and families
  • Coordinate and attend Staff meetings, as well as facilitate trainings
  • Ensure the Creative Works Network (CWN) production, schedule and communication needs are being met.
  • Responsible to hold staff accountable to complete all required agency training and job requirements.
  • Create opportunities that reflect person-centered philosophies and promote social engagement and community inclusion.
  • Encourage independence by providing opportunities for decision-making and choice that are based on personal interests and strengths.
  • Provide outreach to and develop relationships with individuals and businesses in order to create inclusive, opportunities for social and vocational growth.
  • Maintain up to date certifications, required state and agency trainings, including policy and procedure review, as well as attendance at staff meetings.
  • Attendance and punctuality are essential job functions.
  • Follow disciplinary procedures and Address employee issues regarding job performance in a respectful, open and confidential manner.
  • Provide supervision and complete staff evaluations as assigned.
  • Model and maintain a positive, productive team and work environment.
  • Complete time and attendance procedures.
  • Ensure timely reporting of work or non-work related injuries or illnesses to HR Department.
  • Promotes high expectations for staff
  • Upholds a professional standard of personal conduct
  • Projects a demeanor that is welcoming, supportive, and engaging
  • Is engaging and articulate in public forums

Additional Functions of the Job

  • The physical demands are essential functions of the role to safely and effectively perform job duties.
  • Sitting, standing, lifting and carrying 20-30 lbs., reaching, squatting, climbing stairs, kneeling, and moving light furniture may be required.
  • Ability to operate multimedia equipment including current technology. Computer, keyboard, typewriter, calculator, telephone, and other equipment appropriate for this position.

General Requirements

  • Bachelors Degree preferred in special education, human services, psychology or a related field.
  • Minimum of a High school diploma or GED.
  • 5 to 7 year's relevant experience supporting people with intellectual and developmental disabilities.
  • Maintain a valid Driver's License; vehicle inspection, registration and insurance.
  • A clear criminal background check.
