Job Title: Tribal Family Services Prevention Case Manager

Reports To: Social Services Manager
FLSA Status: Non-Exempt
Job Function: Non-clinical
Classification: Regular, Full-Time, 40 hours/week
Schedule: Monday-Friday, 8:00am-4:30pm (must be flexible for home visitations)
Location: Onsite (Valley Center, CA and surrounding area)
Funding: Grant
Pay range: $21.37 to $23.63 per hour DOE

SUMMARY
The Tribal Family Services Prevention Case Manager (TFS Prevention Case Manager) assists Indian families by providing in-home parenting and child abuse prevention services. The TFS Prevention Case Manager will work closely with TFS Court Advocate and San Diego County Health and Human Services Agency (HHSA) to provide information and resources and referrals to prevent the breakup of Indian Families.

ESSENTIAL DUTIES AND RESPONSIBILITIES
Include the following. Other duties may be assigned.
1. Works closely with the TFS Court Advocate and HHSA to prevent child abuse through education, referrals and assessment in San Diego County North Inland Region.
2. Works in Native community with authorities in investigations of abuse and neglect referrals and provides services, education, assessments and referrals to crisis counseling and developmental screenings.
3. Keeps current with community needs for services and prevention health education to recognize the mental health conditions and break down barrier to access services.
4. Conducts community presentations, parent education and child abuse prevention are services available.
5. Works closely with all departments in the clinic and attends and contributes to the ICWA case staffing meetings interacting with public health nurses, therapists and other providers to share resources.
6. Attends and contribute to ICWA Case Management, Child Protective Team (CPT) meetings to support client needs, address community issues and promote resources.
7. Willingness to cross train in other aspects of Tribal Family Services and able to perform Medi-Cal eligibility services.

SUPERVSORY RESPONSIBILITIES
This position has no supervisory responsibilities.

TRAVEL & DRIVING REQUIREMENTS
Travel may be required for this position.
Driving is required for this position. Must be familiar and comfortable driving around our service area and areas where our clients reside for home investigations.

QUALIFICATIONS

EDUCATION AND/OR EXPERIENCE
  • Bachelor’s degree in related field. Master’s preferred
  • Minimum of two (2) years related experience in case management in a culturally
diverse population.
KNOWLEDGE, SKILLS AND ABILITIES
  • Knowledge of ICWA regulations, County CPS agency.
  • Knowledge of HIPAA and Child Protection laws.
  • Knowledge of culturally diverse populations and the willingness to be trained.
  • Skilled in parent education, community presentations and facilitating meetings.
  • Skilled in interviewing, community presentations, facilitating meetings.
  • Skilled in in obtaining information and cooperation from individuals or groups.
  • Ability to establish and maintain effective working relationships with patients, staff and the public.
  • Ability to interpret and communicate social work agency rules, regulations, and procedures.
  • Ability to write reports, correctly reflect treatment and document patient progress.
CERTIFICATES, LICENSES AND REGULATIONS
  • A valid California driver’s license and a good driving record will be required at the time of appointment and must be maintained throughout employment.
  • Current CPR certification or attain within 6 months of hire.
  • Applicants must successfully pass a pre-employment physical exam, tuberculin skin test or x-ray and urine drug screen test (to include marijuana). Health must be adequate to perform all duties of the position.
  • Applicants must be fully vaccinated against COVID-19 and provide proof of vaccination.
  • Must pass background check.
TECHNOLOGY SKILLS
Must be proficient in the following:
  • Productivity Software: Microsoft Office 365; Word, Excel, PowerPoint, Email & Calendar Management (outlook), Teams (video conferencing and meetings), and Share Point.
  • Electronic health records software (NextGen preferred).
  • Utilization of Paycom, Relias Learning, and PolicyTech systems.
