Job description

This position involves conducting UAS assessments to determine service needs and continued eligibility; coordinates with Care Management Team to maintain a high level of efficient and effective individualized Care Management operations.

Duties Include:

• Schedules appointments within 24 hours of case assignment with email confirmation to UAS coordinator for all scheduled appointments.
• Upon completion of the assessment, all cases must be transmitted and completed paperwork must be submitted within 24 hours.
• Conducts quality checks on all completed documents before finalizing and sending.
• Abides by and promotes Agency administrative policies.
• Attends meetings and training as required (webcam or in-person).
• Maintains current and active Health Commerce System access.
• Maintains a continuing knowledge of competencies related to the nursing profession by participating in formal education programs, conferences, workshops, and professional organizations.

Requirements:

• Current RN Licensure in New York State
• 2+ years of RN experience in home care or 1year of RN experience with UAS or MLTC
• Must be fluent in one of the following languages: Korean, Chinese, Bengali, Hindi
• Must be open to working remote and in the field
