Purpose of the Position:
The Virtual Health Coordinator is responsible for the delivery and documentation of Virtual Health visits, the monitoring of the transmission of patients’ biometric information, and intervening when biometric values fall outside of established parameters.

Major Responsibilities:

• The Virtual Health Coordinator is responsible for the delivery of the Virtual Health visits.
• The Virtual Health Coordinator will conduct and document virtual health visits on a select patient population as scheduled.
• The Virtual Health visit is a virtual assessment of the patient’s presentation utilizing clinical assessment skills.
• The Virtual Health visits will include the assessment and evaluation of biometrics via monitor/technology.
• Monitor patient transmission of biometrics and identify variances outside of established parameters and intervene as indicated.
• Assess patients for Program appropriateness.
• The Virtual Health Coordinator is an integral part of the Interdisciplinary Team and coordinates and communicates care and reports findings to the patient’s physician and the Interdisciplinary Team.
• Attends case conferences on Virtual Health patients.
• Is knowledgeable of Program metrics and Analytics.
• Submits data as required regarding patient outcomes and utilization.
• Participates in Virtual Health Program marketing efforts and events
• Position is located in White Plains corporate office

Qualifications:

Education: Bachelor of Science in Nursing

Current, unencumbered New York State Registered Nurse Professional License

Recent experience in a clinical setting assessing body systems required, and a concentration in cardiac assessment preferred

Experience working with tele-monitoring equipment required

Experience working in CHHA preferred

Strong Time Management and Organizational skills

Excellent communication and interpersonal skills

Demonstrates understanding of the mission, goals and objectives of the program

Job Type: Full-time

Pay: $90,000.00 per year
