Great Lakes Home Health Care is looking for an experienced RN to join our growing team. We are new to the area and are excited for the future of the home care industry. We are passionate about giving the very best care to improve patient lives and outcomes.

The Part-time Registered Nurse (RN) provides and directs provisions of nursing care to clients in their homes in collaboration with the primary health care provider, and in compliance with applicable laws, regulations, and agency policies. Also, Registered Nurse (RN) coordinates the plan of care in conjunction with other health care professionals. The home health RN helps to achieve excellent clinical outcomes of care through the delivery of nursing process.

License Requirements

  • Must have current unencumbered RN license for the state of Wisconsin
  • Active unrestricted driver's license and insurance
  • Current CPR certification required.
  • Current Vaccines for Covid-19 or medical exemptions required

Qualifications:

  • 1 year of clinical experience - ICU, ER, Home Health, Hospice preferred, but not required
  • Provide care to patients as outlined by the Plan of Care.
  • Does Supervisory Visits as needed.

Job Types: Part-time, PRN

  • Wound care: 1 year (Preferred)

License/Certification:

  • RN License (Required)
