Monday-Friday

Plan, implement, and maintain a volunteer program to meet the needs of families/caregivers, as well as regulatory requirements for the Medicare Conditions of Participation. Recruit, retain, and supervise volunteers involved in the hospice volunteer program.

  • Key Accountability
  • Develop and deliver educational programs and materials for all volunteers.
  • Evaluate volunteer program services annually and provide recommendations for development, improvement, or expansion.
  • Provide information to individuals and community groups about hospice volunteer services.
  • Prepares monthly and annual service reports.
  • Acts as liaison between hospice staff and volunteers.
  • Participates in quality assurance and performance improvement activities as assigned.
  • Attend regular IDT meetings and complete IDT documentation.
  • Facilitates community awareness and support of UnityPoint at Home volunteer program.
  • Acts as liaison between hospice staff and volunteers.
  • Carry out other assignments as may be appropriate.
  • Participates in quality assurance and performance improvement activities as assigned.
  • Attend staff meetings and other meetings as assigned and appropriate.
  • Participate in orientation and training of new employees working in the hospice program.
  • Prepare service reports as requested by Hospice Administrator.
  • Assures volunteers and volunteer program meets Hospice Conditions of Participation.
  • Enter hospice volunteer frequency orders as needed.
  • Maintain a record-keeping system in SharePoint which includes services delivered and actual time involved.

  • Develop and maintain the orientation and training of volunteers.
  • Recruit, interview, select, and retain volunteers.
  • Assign volunteers to service on the basis of program needs and the volunteers’ interest and skills.
  • Plan and implement volunteer support meetings and recognition.
  • Monitor and evaluate volunteer performance in a timely and appropriate manner.
  • Conducts a minimum of 4 educational components per year for all current volunteers.
  • Assures volunteer’s compliance with agency policies and procedures.

Key Accountability

  • Responsible for completion of communication tasks and activities in a timely manner. This would include, but is not limited to: responding to email, phone calls, or voicemail, promptly, accurately, and professionally; attending staff meetings as scheduled or viewing recordings/minutes of those meetings; asking questions of team members and supervisors when needing clarification about various day-to-day issues or patient needs; and reviewing employee communication pieces, to stay aware of UnityPoint at Home programs and initiatives.
  • Sustain Person Centered Care model by completing formal training on the curriculum. Use Patient Center Care philosophy and tools in clinical practice. Satisfactory achieve acceptable level of performance review annually.
  • Meets measures as determined and required for job productivity and performance improvement.
  • Maintain regular and consistent attendance at work.
  • Behave in a manner consistent with Mission, Vision, Values and Expectations for Excellence.
  • Maintain compliance with OSHA, Accreditation Standards and Risk Management guidelines.
  • Maintain compliance with Personnel policies and procedures.
  • Behave in a manner consistent with all Corporate Compliance policies and procedures.
  • Perform other duties as requested by Clinical Manager or Coordinator to facilitate the smooth and effective operations of the office.

  • Demonstrates ability to meet business needs of department with regular, reliable attendance.
  • Employee maintains current licenses and/or certifications required for the position.
  • Practices and reflects knowledge of HIPAA, TJC, DNV, OSHA and other federal/state regulatory agencies guiding healthcare.
  • Completes all annual education and competency requirements within the calendar year.
  • Is knowledgeable of hospital and department compliance requirements for federally funded healthcare programs (e.g. Medicare and Medicaid) regarding fraud, waste and abuse. Brings any questions or concerns regarding compliance to the immediate attention of hospital administrative staff. Takes appropriate action on concerns reported by department staff related to compliance.

Education:


Associates degree in business or related field or equivalent work experience.


Experience:


Minimum of two years’ experience working with patients/families/caregivers through death and dying issues


License(s)/Certification(s):


Valid driver’s license when driving any vehicle for work-related reasons.


Valid Mandatory Reporter course completion by state(s) requirement


Knowledge/Skills/Abilities:


Mature individual supportive of the Hospice concept.

Previous volunteer experience.

Strong interpersonal skills.

Ability to take initiative and exercise independent judgment, decision making and problem solving expertise.

Ability to work as a collaborative team member.

Ability to understand and apply guidelines, policies and procedures.


Other:
