The Harmony Healthcare Long Island, formerly known as (Long Island Federally Qualified Health Center or LIFQHC) is a non-profit healthcare organization with 7 health centers, providing primary care and preventative medicine in the following locations in Nassau County: Roosevelt, Elmont, Hempstead, Freeport, Oceanside, and New Cassel/Westbury. In addition, the Harmony Healthcare Long Island has 3 school-based health centers, WIC offices (Special Supplemental Nutrition Program for Women, Infants, and Children) in 3 locations, and a Health Home Care Coordination program. As federally qualified health centers, we serve the individuals in our communities, providing enhanced services, expanded hours and reduced prescription pricing, while raising the level of care. JOB TITLE: WIC Clerk

REPORTS TO: WIC Director

The following statements reflect the general duties, responsibilities and competencies considered necessary to perform the essential functions of the job and should not be considered as a detailed description of all the work requirements of this position.


JOB SUMMARY: Acts as receptionist, determines financial, residential and categorical WIC eligibility, weights, measures and records data. Schedules clinic appointments for program participants; assists in nutrition, participant centered activities, and general program operations. Establishes and maintains accurate and accessible office filing system, tracks documents required.


May travel to different service locations as needed. All staff work together to support each other throughout the nutrition assessment and education process. The overall goal is to provide customer-friendly, professional, participant-centered quality care that will enhance the lives of those served.


RESPONSIBILITIES:

  • Collect information for the nutrition assessment process.
  • Promote breastfeeding as the preferred infant feeding method.
  • Complete issuance of food packages that do not require medical documentation, once prescribed by the Nutritionist.
  • Provide and document general participant-centered nutrition education (individual/family sessions and facilitated group discussions) under the guidance of a
  • Nutritionist.
  • Provide referrals to health and public assistance programs, and document follow-up.
  • identify and refer participants to a Nutritionist.
  • Assist Nutritionists in planning and evaluating participant-centered nutrition education.


MINIMUM QUALIFICATIONS:

  • High school diploma or GED and one year of experience working with children and families
  • Bilingual in Spanish and English required


SALARY: Commensurate with experience
